One of the primary functions of our Admission department is to prepare the paperwork on each resident prior to their admission to Mulberry Square Elder Care & Rehabilitation Center. The following is the admission process that must be followed for each referral to the facility.
Pre-Admission:
From the Physician - please authorize the doctor to release the resident/patient medical history to Mulberry Square. This history normally includes medical and surgical information, any recent changes in his or her condition, medication, diet and functional abilities along with nursing care needs.
From the Hospital - please sign the release to authorize the hospital to release medical records to our facility.
Facility Tour - an appointment is not always necessary to receive a tour, however, if you want to see the Admissions Coordinator, please call and schedule an appointment with Melza Kirsch, RN, (814-938-6020).
At the Time of Admission:
The following information will be required: completed admission application; Medical insurance identification card(s): Medicare, Blue Cross, Blue Shield, AARP, Medicaid, PACE, etc. Ambulance information would also be helpful.
If possible, please provide the following:
Advanced Directive, such as a living will.
Power of Attorney documents.
Legal Guardianship documents.
Responsible family member or power of attorney will need to sign admission forms if the resident is unable to do so.
Information regarding Church and Pastor.
Information regarding all next of kin, (names, addresses, and telephone numbers).
Information regarding a Funeral Director and prepared burial papers.
Proof of age:
One of the following proofs of age: PACE card, current or expired drivers license, PA ID, birth certificate, baptismal certificate, passport, or military discharge papers. If you are unable to obtain one of these proofs of age, please call your Social Security Office to request a "Proof of Age" letter. Jefferson County Social Security Office phone number is 800-772-1213. Please bring this letter to the receptionist as soon as you receive it.
Our Social Services Department is a primary link between the nursing center and the family. They get to know each resident and to help him or her to adjust to the facility. They also conduct support groups that meet regularly, in which families can discuss the problems and feelings that arise when a relative enters or resides in a nursing center. Our Social services personnel are always available for private counseling or to address any concerns that may arise.
